Media Release Forms

Before sharing photos and videos of GLOBE events online or with the GLOBE team, anyone recognizable in those photos must sign a media release form. To complete the media release process:

1. Select the appropriate media release form below.
   • Media Release Form for ADULTS (fillable PDF; for ages 18 and above)
   • Media Release Form for MINORS (fillable PDF)
      
2. Fill in both pages of the appropriate media release form, sign, and send back to your GLOBE contact or directly to the GLOBE Communications Team.

Translations

At this time, we do not have translations of the GLOBE Program media release forms. Once the forms are translated, they will be posted here. Please use the English versions above. We apologize for any inconvenience.
