Media Release Forms
Before attending certain GLOBE events or functions, participants must sign our Media Release Forms. To complete the media release process:
- Select the preferred media release form below.
- Media Release Form for ADULTS (fillable PDF)
- Media Release Form for ADULTS (online submission)
- Media Release Form for MINORS (fillable PDF)
- Media Release Form for MINORS (online submission)
- Fillable PDF: Download the form, complete it, and and email it to communications@nasaglobe.org.
- Online submission: Fill out the online form and click Submit.
Important: Make sure to complete both PDF pages of the media release form. Page 1 contains a NASA-specific form while Page 2 contains a GLOBE-specific form.
